This is a list of forms that are requested to be filled in during the first visit.  Clients are invited to print them out here and fill them in and bring them.

Intake Forms Combined	These are all of the intake forms you will need combined into one .pdf file.
Confidentiality Form	This form explains the limits of confidentiality and must be signed before counseling begins.
Registration Form	This is demographic information and releases to insurance companies.
HIPPA Form	This is a government requirement to protect the client's confidentiality.
Presenting Issues Questionaire	This helps to know where to start and how best to focus the therapy.
Personal History	Optional but could be helpful.
Consent to Release Info	This is used if you want information to be released to your primary care physician or other medical professional
